Practice Policy
INITIAL EVALUATION & SESSIONS
​
During the initial session, which typically lasts for 60 minutes, our nurse practitioners conduct an extensive psychiatric evaluation. This assessment is tailored to each patient and helps determine the most suitable treatment plan. To make this evaluation as conclusive as possible, please bring any details relating to previous providers, past psychiatric treatments and medication trials. In some cases, further sessions are required and extra documents such as school reports and family information are important for children and adolescents – and may be helpful for adult patients too. All these topics will be discussed in the first meeting and mutually decided if the nurse practitioner is a good fit for your care.

MEDICATION MANAGEMENT
​
Medications may be used to treat a variety of conditions. It is essential to find the right treatment for each individual instance. If medication is deemed necessary, it is important that you understand the target symptoms and potential outcomes. Your nurse practitioner will explain the risks and benefits, side effects, government warnings and other non-medication alternatives. In some cases, a combination of medication and therapy could be the best course of action. Should this arise, your nurse practitioner can refer you to an outside provider for therapy. Before doing so, they will have a discussion with you about whether or not therapy should be part of your treatment plan so that the most suitable option can be chosen for your needs
APPOINTMENTS AND CANCELLATIONS

The standard meeting time for the initial visit is 45-60 minutes and follow up visits are 15-30 minutes.

Revive Mental Wellness is committed to providing quality mental health care in a timely manner. We prioritize our clients' individual, client-centered needs and have implemented a cancellation and no-show policy to ensure that all available appointments are used efficiently and effectively. Our goal is to provide the best possible care for all of our patients.
We understand that unforeseeable events can arise and prevent you from attending an appointment. To ensure our available appointments are used efficiently, we kindly ask you to provide us with at least 24 hours notice if you must cancel or reschedule an appointment. For your convenience, we count one business day as 24 hours. Missing an appointment without giving 24 hour prior notice is considered a “No-Show”. Being more than 10 minutes late to your appointment will also result in a “No-Show”. We understand that delays can happen, however, it is important to keep other patients and providers on time. If you are running late, please notify our office.

After two such consecutive occurrences in a month (last minute cancellations or no-shows) or recurrences in three months, you will be removed from the schedule until such time you are able to commit to keeping your scheduled appointment time, including being put on a week-to-week basis until consistency and reliability is shown
​
NO-SHOW POLICY
​
Revive Mental Wellness describes “No-Show” appointment as any scheduled appointment the patient either:
1. Does not arrive at scheduled appointment
2. Cancels with less than 24 hrs notice
3. Arrives more than 10 minutes late and is consequently unable to be seen.
​
We ensure our appointments give each patient the right amount of time with the provider. It is important therefore that you attend your scheduled appointment, and arrive 10 minutes beforehand.
​
As a courtesy, and to help patients remember their scheduled appointments, Revive Mental Wellness will send out reminders by email one week prior to the scheduled appointment, by phone call one day prior to the scheduled appointment and by text message four hours prior to the scheduled appointment. If your schedule changes and you cannot keep your appointment, please contact us with at least 24 hours’ notice so we may reschedule you, and accommodate those patients who are waiting for an appointment.
​
A $50 service charge will be charged if a “No-show” occurs.
​
PATIENT PORTAL REQUIREMENT
​
Onpatient is the platform used as the patient portal for Revive Mental Wellness electronic health record, DrChrono. To ensure HIPPA guidelines are being followed, Onpatient is required for all patients of Revive Mental Wellness to use. Onpatient is an online platform where patients can access their private medical records, view upcoming appointments and check-in online for telehealth appointments, manage demographic and insurance information, schedule and cancel appointments, message the provider, request refills for prescription medication, view statements and pay online.

CONTACTING THE PROVIDER
​
To contact your provider, you may call our front office 208-398-3351. If the provider is unavailable, please leave a message. Please allow up to 2 business days for the provider to respond. For easier, faster communication, login to your Onpatient Patient Portal. The provider is able to quickly respond within 1 business day.
​
TELEPHONE ACCESSIBILITY
​
If you need to contact Revive Mental Wellness between sessions, please call our main number or send us a message through the website. We are often not immediately available; however, we will attempt to return your call or message within 24 hours. Please note that Face-to-face video visits are highly preferable to phone visits. However, in the event that you are out of town, sick or need additional support, phone sessions are available. If a true emergency situation arises, please call 911 or go to your local emergency room.

IN CASE OF EMERGENCY
In the event of an emotional, behavioral, medical crisis or life- threatening emergency call 911 or go to the nearest emergency room. BE ADVISED THAT Revive Mental Wellness DOES NOT PROVIDE 24 HR CRISIS SERVICES.
